Part A: A pharmaceutical representative has just arrived at the office of Dr. Joseph Henderson, a board-certified orthopedic surgeon. The waiting room is swarming with patients waiting to see Dr. Henderson, because he was delayed with an unexpectedly complicated lumbar spinal fusion and laminectomy.

The representative is very insistent, almost belligerent, about seeing the physician immediately, even though she did not have an appointment to see him. In fact, the visit was totally unexpected, as the representative had just been in two weeks ago. Last time the representative was in, she gave Dr. Henderson a variety of readily usable and dispensable medications. She has more of the same today—injectable cortisone with Novocain, muscle relaxants, NSAIDs, and even some Tylenol with codeine. Usually, Dr. Henderson is quite receptive to receiving these samples, as they help ease the financial burden on his patients for whom he uses or to whom he dispenses these samples. The office is, in fact, running quite low on these particular medications because of Dr. Henderson’s heavy patient load.

Provide detailed answers for each of the following questions. Your response should be at least 150 words in length.

What is your response to the sales representative?

Should a sales representative ever take precedence over scheduled appointments?

Does the fact that Dr. Henderson is usually quite anxious to receive any and all samples for his patients enter in as a factor?

Does the diminished supply of these samples alter the situation?

Can the medical assistant ever accept delivery of any or all of these samples?

Part B: Dr. Jonas runs a private practice. He admits patients and makes rounds in two local hospitals. He uses one type of EHR software in his private office and two other packages in the two hospitals. Not only must Dr. Jonas learn three software systems, but he also may at times be unable to move patient information between those systems because of incompatibility. What might Dr. Jonas do to address these issues? Your response should be at least 100 words in length.

Part C: Lisa Medina, a certified coder, performs medical coding for a large multi-specialty clinic. You have just been hired as Lisa’s assistant. She has asked you to review the encounter forms for the day, on which physicians have checked off the diagnoses of each patient. You notice that Dr. Parker, an endocrinologist, has checked off the box for Diabetes unspecified for most of his patients without checking off any manifestations or complications. You think this is unusual because many diabetic patients do have complications.

Provide detailed answers for each of the following questions. Your response should at least 150 words in length.

Part D: Sarah Egan is the office manager in Dr. Williams’s practice. Nell Jacobs, who has worked as a CMA (AAMA) in the office for one year, has frequently been absent or tardy on Mondays. Sarah suspects that Nell has a drinking problem. However, Nell has never arrived at the office intoxicated—until today. Sarah has just observed Nell stumbling in the parking lot when getting out of her car. Her speech is slurred, and her breath has a fruity odor that Sarah thinks could be alcohol. Nell does not appear to understand anything that Sarah is saying to her.

Provide detailed answers for each of the following questions. Your response should at least 250 words in length.

Given the situation, as the office manager, what should Sarah do immediately regarding Nell?

If Sarah decides to send Nell home, should she call Nell’s husband to come and get her, or, perhaps, insist that Nell go home in a cab?

After looking at the example and simulated EHR last week it is clear that using the EHR keeps data consistent and easy to access. This improves the quality be allowing for a specific space for each piece of information and giving the indication of what is required for the chart to be complete.

 

The majority of the data that was collected in the demo would be gathered in the CDI program. The chief complaint and ICD 10 or other related procedure and diagnosis codes would be used for “quality reporting, reimbursement, disease tracking, and other administrative and clinical processes” (Monica, 2018). This would allow for higher opportunities of improvement and training.

Hello Professor and Class,

EHR can help improve data quality by being easily accessible. EHR also helps keep patient data more organized and more comfortable to extract. EHR’s also make reoccurring reminders and alerts for patients and physicians. EHR data quality affects the validity of research results, and this is why it’s vital to have good data quality. After last week’s simulation of EHR, I noticed just how important it is to have good data quality. I did not find the simulation user-friendly, and I found a few things very hard to find within the EHR. To have useful quality data can save time for physicians looking and searching for something within the EHR and more time with the patient.

Clinical Documentation Improvement is a program that is created of a dedicated team of healthcare professionals that will make sure that the medical record documentation will reflect the patient’s diagnoses, care provided, and their quality of care provided. In contrast, that patient received care from the provider.

Hello Class and Professor,

Adaptive immunity is two different types of immunity known as cell-mediated and antibody-mediated. Both types of organs can have negative effects when someone has an organ transplant, and both are triggered by antigens. Cell-mediated immunity is when the tissue sustained injury during cell isolation or organ retrieval which gets the adaptive response going. T cells get responses for activation with two of them being antigen recognition and costimulation. These rejections can happen very early on or late after transplantation (Wood, Zaitsu, Goto, 2013). Antibody-mediated immunity is when allograft rejection is caused by antibodies against donor specific HLA molecules, endothelial cell antigens or a blood group antigen. This usually happens right away and can be resistant to therapy and threatens the graft from healing properly and having a good outcome (Singh, Pirsch, Samaniego 2009).

When a rejection occurs, there are a few options for how they will treat it. This well depend though on how bad the rejection is, what type of rejection it is and how severe it is. Sometimes if it happens quickly it can be treated right away rather than someone not knowing that is what is going on and it gets worst. When a rejection occurs, the goal is to suppress the immune system. To do this they will put you on an antibiotic that can either be oral or through an iv. Which they choose and how high of a dosage it is will depend on the severity of the rejection. A lot of times people who have rejection ends up needing to take these medications for the rest of their lives (Dugdale, 2019).

There are four different types of grafts, they include autografts, isografts, allografts and xenografts. Autograft is when you have a skin graft done using skin from your own body to do so. Allograft is when you use skin from a donor’s body to complete the skin graft. Being sure that you have a donor that matches you the best as possible is the most important thing when doing a allograft. The person receiving the graft may also be given anti-rejection meds before hand to ensure that their body does not reject it (Dugdale, 2019).

The skin cell gun is a new thing that was developed in 2008. This is basically a gun that regenerates skin cells. This is done by using it like a paint gun and sprays your own skin cells onto the burn. This is generally used for people who have large burns that cover a good portion of there body. If they can continue to find that this works it will help with healing time, scarring, and reduce infections. This would be huge for second and third degree burns as they are the worst and can take a long time to heal and cause a lot of scarring (Yetman, 2020).

Good morning,

Where I work we work with people coming out of prison, drug dealers, child abuser, and pedophiles Actually we are the only one in Atlantic County, New Jersey that accepts clients with these issues. All the employees are train o how to deal with them and things we can do should someone get out of hand while in a group setting. I have been working with them for about six years and very seldom is there an outburst in the group setting. Most of them are on life parole and do not want to go back to prison.

Recidivism — a relapse or return to criminal behavior — is the biggest challenge that criminals and society face when prisoners are released. Groups that help former inmates after a run-in with the law can reduce these numbers and help the newly released find a meaningful place within society”(Brenner, 2017). They can relapse on drugs and alcohol should they not change, however most come to group because they have to. When release they are they usually get into a half-way house and an intensive outpatient (IOP). Sex offenders and violent criminals, who represent a significant threat to society, cannot be sent to these re-entry centers “(Brenner, 2017). So where do they go? If they do not have family they are sent to the Atlantic City Rescue Mission in New Jersey which is a place where all homeless people go or the are sent to a house where everyone that lives there has a mentally health issues. Such as schziperia, major depression, people who needs monitoring and are mentally challenge.

I believe that everyone should be given a chance at recovery. Some of these clients will never work again because of their conviction. I would to start groups for people that are out of prison and into society, “Groups that help former inmates after a run-in with the law can reduce these numbers and help the newly released find a meaningful place within society” (Brenner, 2017). In group setting or therapy we would start getting them to feel safe and in a non-judgment environment, explaining the HIPPA laws and confidentiality. We would start with asking the client what’s their plan and setting goals. Module 1 Control Substance Abuse

Control Substance Federal Law was created to control the abuse of certain medication that can lead to addiction specially on young adult. Pharmacies and doctor office should have a strict control of the prescription in case to get an audit from the DEA agency but in my opinion it is very difficult to restrict people from getting this controlled schedule II or III once the refill of medicament is not aloud the patient should look for another way to find it even if it is not legal. I heard about a friend who was addicted to Vicodin for a long time and she used to go to the doctor to get the prescription every month until she had to get it on the street so sad.

In Texas people are addicted to Methanphetamines and in Arizona they are addicted to Opium. I heard on the news that since the begining of this year there was a State Law who prohibit the prescripition of this medicament to young people. This is why every states is trying to go beyond DEA because every state have a diferent statistic abouse which drug is creating more addiction.

DEA have to go trought a lot of reseach and different statistic around the country and it will take a long time to change the schedule of the abusive medication.

I will think that this restriction make difficult to patients that really need the medication according to the condition to access the doses and refill becuase the restrictions from DEA. For example there are some patients who suffer from a chronic pain and need strong medication in high doses they are not addicted they need to improve the quality of life.

In the state of Texas it is not permitted to the Pharmacy Technician to offer counseling to any patient. There is a window next to the Pharmacy counter to offer consultation to the customer once they receive the medicine for the first time or in case they have a question for the Pharmacist.

Pharmacology is the base of the Pharmacy site. When we took this class we learned the different medicaments prescribed according to the condition of the patient and the name brand and the generic. how they should be taking and when. Secondary effect on the patient. I would say this is the bible of the pharmacy every work routine.

One case that would be important to apply the Pharmacology knowledge is to avoid any medication error with the prescription because if you read the name of medicine and it does not match with the condition of the patient you can easily find out that there is something wrong and advice the pharmacist. Another case is when there are some alike names sometimes you can listen and write the wrong name but when you verified the condition for what it was prescribe you will get the error.

Pharmacology teach you how to read a prescription specially the Sig instructions this is vital for a Pharmacy Technician to know.

Another case you used Pharmacology knowledge is to identify when the customer is asking for an specific medication and using either the generic or brand name.

In the state of Texas it is easy to go from brand name to the generic medication in fact since the Obamacare was implemented Doctors were encouraging to prescribe only generic .In my opinion it is because the insurance companies do not want to raise their cost. Getting Generic medication is very crucial for people that do not have insurance. I did subscribe to a plan that help you to obtain medicament cheaper at the pharmacy and they ask for Generic all the time. There are some clinics where people go without insurance and they paid for consultation when they send the request to the pharmacy the medicament is most all the time generic in order to get low cost prices for everyone.

I do work at Walgreen and the pharmacy have a card reward program that lower the cost of medicine because every time you pick up your order you get discount and add points to the account. This is very positive because sometimes you paid zero is you have points to redeem,

After being familiar with the task that Pharmacy Technician perform at the Pharmacy , We can conclude that they are the base of the pharmacy operation. It all depend of the experience and level that Pharmacy Technician obtain during years working in the environment . I would say that after years of working at the Pharmacy they should be able to give some counseling about some medication since they work with prescription everyday and they learn about the pharmacodynamics and pharmacokynetic of medicaments but unfortunately Pharmacist are the one who have the year of studies but the Pharmacy Technician have years of experience after assisting Pharmacist everyday.

Module 5

Telepharmacy is a remote pharmacy site that operate by video conference and permit the customer or patients to communicate with the pharmacist and receive counseling about the medications without having to drive hours from the rural areas where they live. It is operated y a Pharmacist and Pharmacy tehcician who will get the prescription electronically and process the medication orders or refills by the computer using the electronic devices like scan machine. It is very convenient and it also have items OTC available.

In my opinion I think it is very convenient for people who live in rural areas and for Seniors who live by themselves and do not have any family member to get the medication. At this time everything is done by phone and computers and everyone knows how to operate the easy application like video conference. I do not see any disadvantage only that you have to get other service like getting an injection or taking the blood pressure or any diabetic test that must be done on site.

I will love to do this kind of service because you will have more time to focus on your duties and I am qualify to work with any kind of electronic system .I think it will be less chance to get any medication errors because everything would be done electronicaly.
