Discussion: Diagnostic Labels as Powerful Communications

A diagnosis is powerful in the effect it can have on a person’s life and treatment protocol. When working with a client, a social worker must make important decisions—not only about the diagnostic label itself but about whom to tell and when. In this Discussion, you evaluate the use and communication of a diagnosis in a case study.

To prepare: Focus on the complex but precise definition of a mental disorder in the DSM-5 and the concept of dimensionality both there and in the Paris (2015) and Lasalvia (2015) readings. Also note that the definition of a mental disorder includes a set of caveats and recommendations to help find the boundary between normal distress and a mental disorder.

Then consider the following case:

Ms. Evans, age 27, was awaiting honorable discharge from her service in Iraq with the U.S. Navy when her colleagues noticed that she looked increasingly fearful and was talking about hearing voices telling her that the world was going to be destroyed in 2020. With Ms. Evans’s permission, the evaluating [social worker] interviewed one of her closest colleagues, who indicated that Ms. Evans has not been taking good care of herself for several months. Ms. Evans said she was depressed.

The [social worker] also learned that Ms. Evans’s performance of her military job duties had declined during this time and that her commanding officer had recommended to Ms. Evans that she be evaluated by a psychiatrist approximately 2 weeks earlier, for possible depression.

On interview, Ms. Evans endorsed believing the world was going to end soon and indicated that several times she has heard an audible voice that repeats this information. She has a maternal uncle with schizophrenia, and her mother has a diagnosis of bipolar I disorder. Ms. Evans’s toxicology screen is positive for tetrahydrocannabinol (THC). The evaluating [social worker] informs Ms. Evans that she is making a tentative diagnosis of schizophrenia.

One essential aspect of writing for audiences that we have left to explore at the end of our FYW sequence is addressing audience needs, values, and expectations in ways that go “beyond” the conventional academic essay (double-spaced, white pages with 1-inch margins). There are many reasons why you as a writer must be flexible and able to “recast,” remix, re-mediate, and/or de-form your central insights, research, claims, and conclusions from the Research Essay (CEL xxvii). 

  • To infuse your approach to issues with different energy, passion, or personality
  • To be generous in explaining or giving more context for ideas that you write about
  • To reach audiences in different modes, technologies, and materials
  • To address different audiences with different expectations in style, tone, language, structure, etc.
  • To offer new perspectives on issues via different genres of writing
  • To de-form or disassemble your previous research for analysis and re-interpretation  

Just as there are many different purposes for remixing your central insights and ideas, there are also many different ways to accomplish these purposes. For this assignment, we look to the “Beyond the Essay” sections of CEL for inspiration:

  • Designing a Tattoo (Ch. 9, p. 296)
  • Creating an Evaluation Form (Ch. 10, p. 330)

For this final assignment, you will use the suggestions above or come up with your own way to unleash your creativity in remixing or going beyond the writing that you completed for the Unit 3: Addressing Issues Research Essay assignment.

Informed consent is obtained after a client receives complete disclosure of all pertinent information provided by the provider regarding the surgery or procedure to be performed. The nurse may obtain informed consent only after the client verbalizes understanding of the potential benefits and risks associated with the surgery or procedure.

Elements of Informed Consent:

  1. The individual giving consent must fully understand the procedure that will be performed, the risks involved, expected/desired outcomes, expected complications/side effects, and alternative treatments or therapies available.
  2. Consent is given by a competent adult, legal guardian or designated power of attorney (DPOA), emancipated or married minor, parent of a minor, or a court order.
  3. A trained medical interpreter must be provided when the person giving consent is unable to communicate due to a language barrier.
  4. The nurse’s role is to witness the client’s signature and ensure the provider gave the necessary information and that the client understood and is competent to sign. If it has been identified that the client does not understand the procedure or is not competent to sign the informed consent, the nurse must STOP the process and the physician must be notified immediately.

A consultation occurs when a professional provides expert advice in a particular area and determines what treatment or services the client requires. The nursing intervention evolves around facilitating coordination with other health care providers throughout the continuum of care in order to protect the client from conflicting and potentially dangerous treatments.

A referral is a formal request for a special service by another care provider so that the client can access the care identified by the primary care provider or consultant. Some examples of referrals include physical therapy, wound care, and hospice. The nursing intervention continues to evolve around monitoring the client’s response and progress.
