Introduction

 

 

A company that manufactures engine components. In terms of production resources, the company has six plants, including one dedicated to assembly. Manufacturing includes a machining phase, a heat and surface treatment phase, and then the assembly of the sub-assemblies that will be integrated into the engines.

The sales department receives customer orders every day. Given the manufacturing lead times (cycles of 8 to 15 months), customers in a hurry can place priority orders, but at the price of an over-invoicing of 20% of the order. The factories then treat them as a priority. Large customers (more than 10 orders per year) are also given priority.

After a technical control of the ordered articles, the commercial management introduces the order into the computer and obtains in output, the breakdown of the assemblies composed in elementary components which belong to two distinct categories:

– Parts manufactured by the company

– Subcontracted parts

 

It then prints a list of “in-house” components for the factories and a list of subcontracted components for the company’s stock management department for supply. This department also manages the bi-monthly supply of raw materials. The global order is transmitted to the assembly plant, which can finalize the order only after receiving all the required components.

 

Changes (amendments) to customer orders can be made after the order has been registered, in order to amend :

– The article codes (in the case of a technical evolution of the article),

– Or the other characteristics of the article (deadline, quantity ordered, …).

The sales department is also in charge of the follow-up of the invoicing. Actual costs are recorded per order received, with management of the number of hours spent (productive hours transmitted by the plants) and the value (purchase price) of raw materials or semi-finished parts from various suppliers (provided by the inventory management department). The invoice is then transmitted to the assembly plant, which also manages the delivery of the order.

Forensic Crime Laboratory in Centerville

The forensic crime laboratory in Centerville employs five drug chemists. The duties of the drug chemist are to perform drug tests for criminal cases. Chemist Linda has worked for the crime laboratory for over 10 years and is the senior chemist. Her record shows that she performed over 60,000 drug analyses, which led to 11,000 convictions.

Other co-workers and the supervisor became suspicious about her high rates of convictions and took a closer look at her examinations, which revealed a virtually inhumane ability to test thousands of cases. She was responsible for testing 31% of all evidence processed in the laboratory, a total of 10,933 cases. Unfortunately, she wasn’t the lab genius she pretended to be. In reality, she faked her education and did not even have a master’s degree.

The supervisor’s investigation in numerous cases revealed that she faked the test results of thousands of tests per year. It was her way of getting involved in pursuing justice and to give the district attorneys what they needed for guilty verdicts.

When she was questioned by the supervisor, she admitted adding to a defendant’s guilt when she added cocaine to the evidence sample to provide incriminating results. Linda committed not only unethical, but also criminal acts and was arrested by law enforcement. The supervisor notified the district attorney, who had to revisit all the cases she had analyzed and dropped over 1,100 cases, resulting in innocent people and maybe criminals being released from prison and jail.

Based on Linda’s case, assess the ethical obligations of a forensic scientist to follow procedures and legal protocols when examining physical evidence. Critically analyze the duty of truthful courtroom testimony, including perjury. Be sure to address the following in your assessment:

  • Analyze the possibility that forensic scientists give less than truthful testimony in court if it will help convict a dangerous criminal defendant.
  • Create a plan concerning how to improve quality control in state laboratories for drug testing, serology, and fingerprinting analysis. Implement recent research of current programs and improvements in forensic examinations to guarantee due process of the law.
  • Articulate if certification and mentorship increase and promote ethical behavior when performing forensic examinations.
  • Determine if crime laboratories should not be part of the police department and law enforcement and if they should have managed independently to increase ethical behavior by eliminating dependency on and supervision by law enforcement and prosecution. Consider current research on independent laboratories and law enforcement sponsored laboratories and statistical information on the unethical behavior of employees in evidence testing and testimony in court.

the latest is developing apps with AI.  most businesses will have that in a few years if not already.  the most common form of apps are ones that we are using but not quite know              how they do it.  most AI tools are simple and cheap.  but as a business, you need to    decide what platform should be used to be more consistent within the organization.

Pureplay meaning the Amazons of the world, just one channel, Clicks & Bricks meaning the Barnes & Noble, Walmart of the world. The concensus seems to be the Clicks & Bricks are winning in most industries, so far. You can do some research on your own and post any comments you have here regarding the two models. In particular, how should Clicks & Bricks utilize the fact that they have both channels by creating new services.  What about the grocery industry?  I tried Instacart when the virus first hit.  I think this model does not really work, too expensive. if you include fees and tips, it can be up to $20 for a $100 purchase.  as soon as this is over, I am going back in  in person for sure.   how often do you buy from Amazon as vs. walmart etc.   .

as much as we think we are buying everything online, we are not.  before the virus, we were buying about 11% of retail sales online. SO there are still a lot of foot traffic.  Now it is more but that is due to the virus. people like me will go back i person when this is over.

Evidence-based practice is the combination of clinical expertise, patient values and their preferences and the most up-to-date research that is available (Dean, Chapter 3: Practice and Competency Development). Michael Scriven and Richard Paul describe critical thinking as “the mental active process and subtle perception, analysis, synthesis and evaluation of information collected or derived from observation, experience, reflection, reasoning or the communication leading to conviction for action” (Papathanasiou, Kleisiaris, Fradelos, Kakou, & Kourkouta, Critical thinking: The development of an essential skill for nursing students 2014). Critical thinking focuses on intuition, problem solving, knowing what information is and isn’t important, what to prioritize, and the advantages and disadvantages off all treatment options.

My personal opinion is that evidence-based practice is nothing without critical thinking. You really need to be able to think critically to be able to successfully perform evidence-based practice. You may walk into a patient or situation and think that they have one thing because that is what the first couple of major problems point to. However, if you think critically and dive deeper into all their problems, labs, medications, subjective and objective data you receive during your assessment you may quickly realize that there is a different problem. They may be presenting with something like hypoglycemia, but the real problem is something like a tumor on the pancreas, hormone deficiencies, dumping syndrome or anorexia.

As a nurse using critical thinking and evidence-based practice are very important. It allows us to treat each patient individually, take into count new and previous information, think outside the box when the current plan of care if not working, be flexible, assess the reliability of all of the data collected, both subjective and objective (Papathanasiou, Kleisiaris, Fradelos, Kakou, & Kourkouta, Critical thinking: The development of an essential skill for nursing students 2014). By using critical thinking and evidence-based practice we can provide a safe, effective and positive patient outcome.

The purpose of this discussion is to describe a patient case from my clinical practice within the last five years and talk about factors that may have influenced pharmacokinetic  and pharmacodynamic processes of the patient. I will also explain the personalized care plan that was developed for this patient based on these factors and their history.

Pharmacokinetics can be defined as the study of drug movement throughout the body, meaning what the body does to the drug (Rosenthal & Burchum, 2021, p. 13). There are four basic concepts that go along with pharmacokinetics: absorption, distribution, metabolism, and excretion (Phamacology – Pharmacokinetics (Made Easy), 2015). This is how we as practitioners know how to maximize the benefits a drug will have on a person and minimize the harm it may cause an individual. We must know the proper dosage of a medication based off of the patient and patient’s medical history that may delay absorption, metabolism, or excretion of the drug and find the right balance to elicit the proper response. If this is not done properly, then harm could be done to the patient if the balance is off, such as too much medication is given or not enough to benefit the client. Pharmacodynamics can be defined as the study of the biochemical and physiologic effects of drugs on the body and the molecular mechanisms in which those effects are produced, or what the drug does to the body (Rosenthal & Burchum, 2021, p. 22). This is important to understand as a practitioner so we are prescribing the minimal dose needed of a medication to take the desire effect on the patient and how much the medication can be increased in order to elicit the desired effect without causing harm.

A patient that stands out in my mind is a 23-year-old male that was a frequent flier of the emergency department who often came in highly intoxicated, hypoglycemic, sometimes suicidal, and at times unresponsive due to both hypoglycemia and his chronic alcohol abuse issues. This patient and his father both frequented our emergency department, although his father passed away a few months before the patient in this case scenario, he also eventually succumbed to his  alcoholism. The patient was well known to us as having type 1 diabetes and was supposed to be on Novolin R, Lantus (of which he admitted he hadn’t taken in years) and also on 50mg of Zoloft for depression and anxiety. The patient suffered from alcoholism and would binge drink, which would interfere with his diabetes and his insulin regimen, if he remembered to take it, or at times he would become overconfident and just take a random dose of short-acting insulin as he was drinking without checking his sugars. Therefore, we often saw him in different stages hypo or hyperglycemia depending on if he was drinking or just not taking his insulin properly. The patient also suffered from alcohol-induced hepatitis and impaired renal function at his age due to his failure to control his diabetes and his binge drinking for many years.

Insulin’s purpose is to regulate glucose metabolism and this is done by binding to insulin receptors on muscles and adipocytes and by lowering blood glucose by facilitating the cellular re-uptake of  glucose while also inhibiting the output of glucose from the liver (“Insulin, regular (HumuLIN R, myxredlin),” 2021). Insulin does have a short half-life of 86-141 minutes when given subcutaneously and the peak time may vary depending on the brand he was using. Insulin is metabolized within the liver, kidneys, and fat and 30-80% is excreted within the urine, meaning those with impairment to the liver or kidneys, such as with this patient who had a history of hepatitis and impaired kidney function with chronic elevated creatinine and BUN levels, he was unable to properly metabolize or excrete the insulin he was giving himself, causing his blood sugar levels to be off and hypoglycemia to occur due to alcoholism (Pastor, Conn, Teng, O’Brien, Loh, Collins, MacIsaac, & Bonomo, p. 188).

As the care team, we had to try and come up with ways to care for this patient and his chronic issues taking into account his patient factors, such as young age, being a male and more at risk for hereditary alcoholism, improper diabetic diet, alcohol abuse, and his risky behaviors of too much or too little insulin without checking his sugars to verify the dose needed. We consulted endocrinology many times for this patient, heavy patient education on the proper insulin dosages, long and short acting insulin and also the need to check his sugars multiple times a day, had a dietician see the patient as well and attempted to get the patient into a rehab facility. The patient was not agreeable to changing his lifestyle and thought he was “invincible”. This patient did eventually pass away from his lifestyle choices and other co-morbidities at a young age.

As clinicians, we need to do more for our patients to advocate for them from the beginning and start screening our patients from a young age especially when there is a family history of alcoholism. We also need to be monitoring our patient’s medications more closely, as this patient should not have been so free with his insulin and had better education and guidelines from day one, maybe it may have engraved a habit in him. This is why it is so important for practitioners to also understand the pharmacokinetics and dynamics of a medication, in someone like this patient, who needed different monitoring and dosing of his insulin due to his liver and renal impairments and inability to properly metabolize and excrete the insulin.
