Defining the Community

Your community can be any geographically defined county, city, or town. Clearly delineate the following dimensions before starting the process of community assessment:

  • Describe the population that is being assessed?
  • What is/are the race(s) of this population within the community?
  • Are there boundaries of this group? If so, what are they?
  • Does this community exist within a certain city or county?
  • Are there general characteristics that separate this group from others?
  • Education levels, birth/death rates, age of deaths, insured/uninsured?
  • Where is this group located geographically…? Urban/rural?
  • Why is a community assessment being performed? What purpose will it serve?
  • How will information for the community assessment be collected?

Assessment

After the community has been defined, the next phase is assessment. The following items describe several resources and methods that can be used to gather and generate data. These items serve as a starting point for data collection. This is not an all-inclusive list of resources and methods that may be used when a community assessment is conducted.

The time frame for completion of the assessment may influence which methods are used. Nonetheless, these items should be reviewed to determine what information will be useful to collect about the community that is being assessed. It is not necessary to use all of these resources and methods; however, use of a variety of methods is helpful when one is exploring the needs of a community.

Demographics of the Community

  • When demographic data are collected, it is useful to collect data from a variety of levels so comparisons can be made.
  • If the population that is being assessed is located within a specific setting, it may be best to contact that agency to retrieve specific information about that population.

After data are collected from various sources, it is important to review the information and to identify assets and areas for improvement in the community by comparing local data (if available) versus state and national data. This will facilitate organization of the information that has already been obtained and will provide direction for the next step of the process.

Windshield Surveys

With the use of public transportation or by driving a vehicle around the community, one can observe common characteristics of the community.

Examples of key observations to make when one is assessing the community through a windshield survey include the following:

Transportation in the community

Focus Groups

Focus groups (usually small groups of 6-12 people) can be helpful when one is gathering information about specific areas of concern within the population. Use of a focus group involves open dialogue about the population, whereas an interview or survey yields only individual responses.

Community resources used

Transportation issues within the community

Safety within the community

Surveys

Surveys may be used to collect data from the community. Selecting a sample of the target population may prove helpful in the collection of data that are easier to analyze. It is important to ensure that the sample is representative of the target population.

A survey should be developed that takes into consideration the developmental level of the group that is being assessed. Questions should be written at the appropriate developmental level, so they are answered in a way that makes the data useful. Surveys might include closed-ended (yes/no), multiple choice (several responses to choose from), Likert scale (Strongly Agree/Agree/Neutral/Disagree/Strongly Disagree), or open-ended (“why”/“how”) questions.

For example, if you have ever read a research article, then you have used health care data because the researcher has used health care data to perform the research. You do not necessarily have to have personal experience using this data. Remember you are “in the users shoes” and need to describe how they would use the data. Your answers should be specific. Not just “doctor gives care.”

I’ve included a template in Excel for you to use for your assignment (see attached). I’ve also given you an example of what it should look like. If you are not sure how to use Excel, go to the FAQ link on the left to find some short videos.

The purpose of this assignment is to give you an idea of how challenging it is when health care is a scarce resource. There will never be enough money for everyone to have every health care service they may need or want. Who makes these decisions? Perhaps a government agency, perhaps an insurance company. It is easy to blame a nameless, faceless business for denying care. But what if it was you? What if you worked in that field and the burden to allocate too few resources to too many needy patients sat on your shoulders? Might you have a completely different perspective? How do payors determine what to cover? The answer is simple and complex at the same time. Payers generally do not cover anything experimental. They have clear policies for what specific plans cover. They often require preauthorization to ensure that the test/procedure is truly necessary and that it is supported by best practice showing improved quality outcomes.

In this case you are not working for an insurance company. You are a senior executive in a hospital. The hospital sets aside charity care monies each year to assist needy patients. There are always more needy patients than there are funds. When the funds are gone, they are gone. This year, you are tasked with distribution of the money.

None of the patients have insurance or any significant funds. You have 1 million dollars. It cannot be used for more than one patient.

The patients are as follows:

1.     Patient A is a 70 y/o retired business executive who needs a $500,000 heart transplant and $500,000 in post care surgery (i.e. medications, rehab, follow- ups, etc.)

2.     Patient B is a toddler with cerebral palsy (affect a person’s ability to move and maintain balance and posture) who needs $500,000 for ongoing care. With expert care the expected lifespan is teenage.

3.     Patient C is a 32 year old mother of 2 who needs a $750,000 bone marrow transplant and $500,000 in post care surgery (i.e. medications, rehab, follow- ups, etc.).

4.     Patient D is a 38 y/o athlete who needs a hepatitis C drug that costs $30,000/ month. The patient could live a normal life span as long as the drug is provided.

5.     Patient E is a micro-premature baby who is costing $10,000 day with a 20% chance of survival. The hospital stay is expected to be 12 months minimum, not including any needed surgical procedures, and not including likely life-long health care needs if he survives.

Determine the distribution of health care funds

Include the following aspects in the assignment:

Ø  Which patient should receive the funding and why?

Ø  If the funds were available to be shared among the patients, would you have chosen differently?

Ø  How did you feel having to make a choice?

Ø  Discuss if this exercise has changed your view of the challenges behind paying for health care.

Write a 1000-1500-word APA paper addressing each of the following points. Be sure to completely answer all the questions for each bullet point. Separate each section in your paper with a clear heading that allows your professor to know which bullet you are addressing in that section of your paper. Support your ideas with at least three (3) outside sources and the textbook using citations in your essay. Make sure to cite using the APA writing style for the essay. Case Study:

Mrs. Adams, a 68-year-old widow who was referred to case management upon discharge from the hospital based on her physician’s recommendation that she is not able to care for herself independently. Her diagnosis is diabetes, hypertension, and breast cancer. She is 5 days’ post-op from a right sided mastectomy.   Mrs. Adams apartment is located in a low income area of the city where crime is prevalent.  Upon assessment by the Community Health Nurse, Mrs. Adams apartment was in disorder with minimal airflow or light. Her cloths appeared unchanged and she had no food in the apartment.  The small apartment also housed 3 cats and a small dog who Mrs. Adams considers family since the death of her husband 1 year ago. Mrs. Adams complains of pain and draining from her surgical site and a broken air conditioner.
