Referral and consultation NP

I’m trying to learn for my Health & Medical class and I’m stuck. Can you help?

Patient care hinges in part on adequate and timely information exchange between treating providers. Referral and reply letters are common means by which doctors and nurse practitioners exchange information pertinent to patient care. Ensuring that letters meet the needs of letter recipients saves time for clinicians and patients, reduces unnecessary repetition of diagnostic investigations, and helps to avoid patient dissatisfaction and loss of confidence in medical practitioners.

As a Nurse Practitioner (NP) you will need to know the difference between a consultation and a referral for treatment, when ordering and when carrying out consultations or referrals.

Consultations

A consultation is a request for opinion or advice, so that the requestor can manage the patient. A consultation is billed under one of the consultation codes listed in Physicians’ Current Procedural Terminology (CPT) (99241-99245 for outpatient of office consultations). If the NP is the consultant, the NP should document the request for a consultation, the reason for the consult, and the NP’s evaluation and recommendations.

When an NP requests a consultation from another provider, the N P should request “consultation” on the referral form, rather than “referring.”

Referrals:

A referral is made when the referring provider wants to turn the management of the patient over to the referred-to provider, at least for the current complaint.

When a NP refers a patient, the NP should state on the referral form that the NP is “referring the patient for evaluation and treatment.” The referred-to provider will bill an evaluation and management code, rather than a consultation code.

Writing Assignment: Consult: Write up a consult request and include all key elements.

Ms. Perez has been referred to Ms. Wilson FNP-C,APRN, MSN for consultation regarding eczema unresponsive to treatment in the past six months.

Document the evaluation and recommendations for how Ms. Wilson FNP-C,APRN,MSN should deal with the consultation request and bill a consultation code.

Since the 2016 presidential election, there have been multiple changes to the Patient Protection and Affordable Care Act (ACA).  What are they and how have they impacted healthcare access, cost, and quality?

A common task in healthcare involves a team assessing how changing regulations, mandates, or market conditions will impact an organization.  At times, the very survival of the entity depends on this analysis.  Each student will be assigned to a team and tasked with working together to develop a presentation on the following topic:

Since the 2016 presidential election, there have been multiple changes to the Patient Protection and Affordable Care Act (ACA).  What are they and how have they impacted healthcare access, cost, and quality?

Questions to be addressed in your research include:

·                  What is the status of those who were newly covered?

·                  What are exchanges and how are different States approaching them?

·                  What does the Medicaid program have to do with the ACA?

·                  How have health insurance premiums been affected?

·                  What variables are in play in the political debate over the impact of the ACA on cost, access, and quality of care?

·                  What are some of the key challenges facing state and national policymakers given the current status?

·                  On balance, has the ACA been a good or bad law?  Provide a clear, fact-based, non-emotional justification for your conclusion.

On balance, is the ACA a good or bad law? Provide a clear, fact-based, non-emotional justification for your conclusion.

Instructions:

Team projects occur in almost all health care worksites. This project encourages student-to-student interaction, the use of technology, and learning. You will be assigned to a team by the beginning of Week 2.

Your project will take the form of a 10 – 15-minute multimedia presentation. You will publish your presentation to an Internet site (like Google, YouTube or your own website) and post the link to the project in the Group Presentation Assignment folder. Your presentation may contain: a PowerPoint Slideshow, a website, a video… the choice is yours. Be as creative as you can. An associated transcript with all the references is required.

Here are some suggestions for creating and posting your project:

If you are into videos and YouTube, you likely already know how to create a video and post it to YouTube. Mac computers come with iMovie and PCs come with MovieMaker; both are easy to use tools for editing video. You can use a video camera to record your presentation. You don’t need anything fancy; the webcam on your laptop or even your phone will record video appropriate for this project. You could use a screen capture software to record the screen activities of your laptop. Quicktime Pro and Camtasia are solutions you can use for screen capture. Once downloaded and running on your machine, they allow you to record screen activity and save that recording as a movie file. There are many screen capture options available and some of them are either free or come with a free 30-day trial. Here is a web article that reviews 17 different free solutions for capturing your screen. Recording a video with either a camera or screen capture, then editing it with iMovie or MovieMaker is what I would suggest.

Another free screen capture program, not listed at the link above, is Easyvid:  http://www.ezvid.com/.  It is well worth a look.

If you are not familiar with any of this, you can use PowerPoint to make a movie. Simply produce a slideshow in PowerPoint and record narration/timing in the slideshow settings. When you save the file, save it “as a movie”. This will create your movie file.

Uploading your presentation to YouTube is also fairly simple. If you do not have a YouTube account, it is free and simple to set up. All you need is an email address. In fact, if you have a Gmail account, you already have a YouTube account. Go to Youtube.com and register for a free account. Once you sign in, there will be an “Upload” button at the center top of your screen. Click upload and locate the movie file you saved. Be sure, in your YouTube settings, to make the movie either “Public” or “Unlisted”… No one but you will be able to see a movie that is set to “Private”. Once the movie is uploaded, navigate to it and copy the link in the address bar. Post the link to your presentation in the Team Presentation folder.

If you have trouble with any of this, there are many videos on YouTube and other tutorials on the Internet that will give you great ideas. Have fun with this assignment and be creative. You are not limited to just making a movie and posting it to YouTube. You may want to try an interesting presentation tool called Prezi. This tool allows you to create very fun, interesting, online presentations. You can even invite collaborators to create your Prezis with you. Check out the site and I encourage you to look for new and different ways to present your ideas. You can give your 10 – 15-minute presentation in any creative way you can think of.

A Family Divided

A Case Study by Robert L. Potter

Discussion Questions by Rosemary Flanigan

A forty-five-year-old man with a three-year history of cardiovascular disease has entered the hospital with a stroke that has paralyzed his right side and caused him to aspirate food of any consistency. His mental status is clouded and there is disagreement as to whether or not he has decisional capacity. His language capacity is only “yes” and “no,” and his responses are inconsistent. The attending physician is convinced that the patient has lost decisional capacity while two family members are equally convinced that he has decisional capacity. The patient’s wife and two other children are ambivalent about his competency to make decisions. The prognosis for recovery of safe swallowing and speech approaches zero because of the dense damage to the cerebral cortex visible on brain imaging. Two neurological consultants have verified that recovery is likely to be minimal and that permanent, severe disability will be the outcome.

The patient does not have an advance directive. The patient’s wife says that they never did discuss his preferences about life-sustaining treatment. She is convinced that he would not want to live in this disabled condition, but is uncertain whether to request the placement of a feeding tube. Two of her four adult children are strongly opposed to the tube placement, while the other two insist that not to do so would be to “kill our father.” The patient’s wife is torn between these two positions, but finally requests that the tube be placed.

The attending physician and the rest of the treatment team are opposed to placing the feeding tube. Their argument is that the patient has “minimal consciousness” and will not improve. They define this as a futile situation with no reasonable expectation of recovery. Furthermore, two nurses claim that during previous hospitalizations for episodes of cardiovascular events the patient told them that he would not want to be sustained by artificial means — not by ventilators, renal dialysis, or tube feeding. It is their position that the patient has expressed his preference to not be kept alive in a futile situation.

The family requests an ethics consultation.

Questions for discussion

1. The judgment of futility should be a conclusion of a communications process, not a beginning. How would you propose beginning the communication?

• Who should be present?

• What information should be shared?

• What alternatives are on the table?

• What is your goal in participating in the discussion?

2. The family is divided over whether or not their husband/father should be placed on a feeding tube. Consider and discuss the following statements:

• The effectiveness of the feeding tube is a clinical matter.
• The benefits of the feeding tube for the patient depend on the patient’s wishes and quality of life, which the patient or the patient’s surrogate and family must determine.
• The burdens the treatment (feeding tube) imposes are a consideration for both parties.
3. Role play the discussion. Express your assumptions and loyalties and listen attentively to the other speakers.
4. Evaluate the role playing experience. Did it affect your view of the case?
If yes, how will you advise the family and the healthcare providers?
If not, what is to be done now?

5. How would this case have been different if the attending physician had been the one asking for a consultation?

Park, M., Cho, S., & Hong. (2015). Prevalence and perpetrators of workplace violence by nursing unit and the relationship between violence and the perceived work environment. Journal of Nursing Scholarship, 47(1), 87-95. doi: 10.1111/jnu.12112

Qualitative Article:

Turk, M., Fapohunda, A., & Zoucha, R. (2015). Using photovoice to explore Nigerian immigrant’s eating and physical activity in the United States. Journal of Nursing Scholarship, 47(1), 16-24. doi: 10.1111/jnu.12105

Summary of the Article

The study postulates that workplace violence is a universal problem that affects many individuals at multiple levels. The authors, Park, Cho, and Hong assert that workplace violence not only has an impact on a person’s quality of life, but also has the potential to affect their emotional as well as physical wellbeing. Every occupation is affected by workplace violence; however, nurses are at a bigger risk especially of non-physical violence. Researchers believe that there is a relationship between violence and work environment. A cross-sectional study was conducted using data collected over a period of two months from nurses at a university hospital in Seoul, South Korea. The 2nd version of the Copenhagen Psychosocial Questionnaire was utilized to examine the potential factors related to violence. The data was computed to establish the number of nurses exposed to one of the five forms of violence over a period of twelve months. Multiple regression analysis was used to identify a commonality among work demands, trust, and justice existing at the workplace. It was established that 71% of the nurses who took part in the study reported previous exposure to at least one of the five forms of violence. The majority reported exposure to verbal abuse as well as sexual harassment. According to the data, most perpetrators of violence are patients followed closely by physicians and patients’ families. Emotional instability potential increased the risk of workplace violence. Researchers established that more attention must be given to address the high workplace violence by building trust and justice, and having the appropriate workload in the workplace. The researchers concluded that nurses exposed to increasing work demands and minimal trust and justice in their work environment are more likely to experience workplace violence. Therefore, reducing workplace violence creates a better work environment and provides the opportunity for better results.

According to the researchers Turk, Fapohunda, and Zoucha, the population of the African immigrants is one of the fastest growing populations in the United States. Nigeria is one of the most popular countries for African immigrants. It is possible that this population may face barriers, making it difficult to maintain healthy diets and activity levels after migrating to the United States. To more knowledge surrounding the African immigrant population, researchers conducted a qualitative visual ethnography study using Photovoice. The 13 participants were asked to describe their health and life experiences using photographs. The participants had to be at least 18years of age, able to read and write, and must have been an immigrant from the Nigeria to the US. Participants were provided with a digital camera and instructed to takes photos of unhealthy eating habits and physical activity that they perceived to be unhealthy over a two-week period. The participants were instructed to take photos of what they thought as healthy eating habits and physical activity. The top four photos were selected and discussed. A semi structured interview guide was used to ask the participants several questions in relation to the photos that were previously selected. Open ended questions were used by the participants to fully express their perceptions. Through data collection, four themes were identified: moderation is healthy, Nigerian lifestyles are healthy, adapting to the American way is unhealthy, and promoting healthy behaviors involves cultural consideration (Turk, Fapohunda & Zoucha, 2015).

The Problem for each article

Workplace violence has become a global problem affecting numerous individuals on multiple levels. Workplace violence not only has an effect on a person’s quality of life, but also impacts their emotional as well as physical wellbeing. All occupations are affected by workplace violence but nurses are at higher risk. Potential risk factors create avenue for workplace violence by various perpetrators (Park, Cho & Hong, 2015).

The health and wellness of African immigrants in the United States has not been examined. Few studies focus on the heath of this population, as well as the possible impact diet and physical activity can have on Nigerian immigrants to the U.S. There is little information available on their perceptions and behaviors involving eating healthy and increased physical activity.

The purpose statement for each article

According to the authors, the purpose of the article is to identify both the prevalence and perpetrators of workplace violence involving nurses. The article seeks to examine the relationship between work demands, truth, and justice within the workplace and workplace violence (Park, Cho & Hong, 2015).

The purpose of the study were to (a) explore the perceptions and practices of Nigerian immigrants regarding healthy eating and physical activity in the United States; (b) assess the influence of cultural beliefs of Nigerian immigrants on eating and physical activity; (c) describe the role that healthcare providers can play in helping to promote healthy eating and physical activity; and (d) evaluate the feasibility and efficacy of using Photovoice to collect data on the perceptions and practices of Nigerian immigrants regarding healthy eating and physical activity” (p. 16).

The hypothesis and/or

Research questions depending on the methodology used in the articles.

There is a relationship between violent occurrences and the work environment; specifically, the existence of trust and justice when such events occur and the demands of the work environment.

Because this is a qualitative study, there is no specified hypothesis. The study addresses the following questions:

What are the perceptions and practice regarding healthy eating habits and physical activity for the Nigerian population? What role do cultural beliefs play in healthy eating and physical activity? What is the role of the healthcare provider in ensuring healthy behaviors regarding eating and physical activity among the Nigerian immigrant population?

The significance of each article to nursing practice.

It is evident in this article that work place violence exists in different forms in almost every aspect of nursing. In order to reduce these patterns, awareness must be raised on workplace violence and possible contributors. The study highlights that apprentice nurses have a higher risk of experiencing violence from their colleagues and shows that it is imperative for professional nurses to get more intentional in eliminating these patterns that undermine the environment of trust and justice (Park, Cho & Hong, 2015).

As professional nurses, it is crucial to understand behavior and health beliefs associated with other cultures. Understanding these cultural behaviors and beliefs allows enhanced communication and education with culturally diverse populations, a vital element in the nursing practice and in enhancing cultural competence. Without having a cultural understanding, it becomes difficult to provide quality patient care. This study revealed that cultural context is an important aspect in promoting healthy behaviors (Turk, Fapohunda & Zoucha, 2015). In order to promote these healthy behaviors, as professional nurse we must take the time to become more culturally aware.

Details supporting whether the study is quantitative or qualitative

Used a structured technique of collecting data; in this case, the questionnaire.

Utilized numeric information for its statistical variables to form conclusive and descriptive analysis.

The authors assert that the research is a “Qualitative visual ethnography using Photovoice.” (p. 16).

In this article, photos were selected and the participants provided descriptions about their experiences regarding the photos. Participants provided descriptions through a series of questions provided during the interview.

The study involved live experiences regarding perceptions and practices relating to healthy eating habits and physical activity of the Nigerian immigrants.

I’m studying for my Health & Medical class and need an explanation.

To prepare for this assignment view the following brief video from the American Medical Association titled, “Health Literacy and Patient Safety: Help Patients Understand.” The video can be accessed through the following link:

https://youtu.be/cGtTZ_vxjyA

Part I: Pamphlet

  1. Develop a pamphlet to inform parents and caregivers about environmental factors that can affect the health of infants.
  2. Use the “Pamphlet Template” document to help you create your pamphlet. Include the following:
  3. Select an environmental factor that poses a threat to the health or safety of infants.
  4. Explain how the environmental factor you selected can potentially affect the health or safety of infants.
  5. Offer recommendations on accident prevention and safety promotion as they relate to the selected environmental factor and the health or safety of infants.
  6. Offer examples, interventions, and suggestions from evidence-based research. A minimum of three scholarly resources are required.
  7. Provide readers with two community resources, a national resource, and a Web-based resource. Include a brief description and contact information for each resource.
  8. In developing your pamphlet, take into consideration the healthcare literacy level of your target audience.

Part II: Pamphlet Sharing Experience

  1. Share the pamphlet you have developed with a parent of an infant child. The parent may be a person from your neighborhood, a parent of an infant from a child-care center in your community, or a parent from another organization, such as a church group with which you have an affiliation.
  2. Provide a written summary of the teaching / learning interaction. Include in your summary:
  3. Demographical information of the parent and child (age, gender, ethnicity, educational level).
  4. Description of parent response to teaching.
  5. Assessment of parent understanding.
  6. Your impressions of the experience; what went well, what can be improved.

So there is 2 documents. one is the pamphlet itself and the second written summary document.

Citations required in APA format not older than 5 years. I am attaching the template for the pamphlet here too.

As you read the content, do you see more differences or similarities in these articles? Explain.

I’m working on a Health & Medical exercise and need support.

Instructions: Review the following articles and, based on your review, answer questions 1-10 from Sections 1 & II. Develop your answers to these questions before you begin the assessment. Once you are ready with the answers, begin the quiz and type or copy your answers in one attempt. There is no word limit for each answer. However, be sure to include key areas to get full credit. Each question is worth 5 points (Total Points: 10 questions x 5 = 50 points).

Section 1. Review the quasi-experimental study entitled, “A pilot evaluation of a social media literacy intervention to reduce risk factors for eating disorders” and the phenomenological article entitled, “We walk on eggshells”: A phenomenological inquiry of wives’ experiences of living with active duty Marine husbands with PTSD.” Based on your review, answer the following questions.

As you read the content, do you see more differences or similarities in these articles? Explain.
In your opinion, why is the quantitative research study titled “A pilot evaluation of a social media literacy intervention to reduce risk factors for eating disorders” labeled as quasi-experimental research, instead of experimental research? Explain.
In the quasi-experimental article, on pg. 849, the author has mentioned the term “measures.” In other words, those were the outcome measures after implementing the social media literacy program. Outcome measures (outcome variables) are known as dependent variables. As a student researcher, you would be interested in knowing the meaning of treatment or intervention (independent) variable(s). Describe the independent and dependent variable(s) used by the researcher in this study.
The research study article titled “We walk on eggshells” A phenomenological inquiry of wives’ experiences of living with active duty Marine husbands with PTSD” has used a phenomenological approach. In your opinion, did the researcher use the right research approach vs. other qualitative approaches? Describe in relation to other approaches OF qualitative studies.
Review the steps of the research process in module 2 and module 3 in Sakai and from Grove, Gray & Burns textbook (Chapters 2, 3). Examine the steps in relation to the quasi-experimental article (A pilot evaluation of a social media literacy intervention to reduce risk factors for eating disorders). Did the author deviate from the steps of research process? Explain.
Explain in your own terms how the results/analyses sections are different in these two (quantitative and qualitative) articles.
Section II. Review the systematic review article titled, “Role of fruits and vegetables in adolescent cardiovascular health: a systematic review” and answer the following questions.

What was the purpose/rationale for the systematic review? Was the rationale and importance for the review stated in relation to previous research and reviews? Explain.
Did the author use a P-I-C-O research question to guide the systematic review? Based on the EBP module review, mention the PICO components that you infer from this article.
What databases and resources were used for retrieving the data? Was the search strategy adequate and appropriate for the review question?
What were the inclusion/exclusion criteria for selecting studies in generating high level of evidence? Why do researchers include inclusion/exclusion criteria in systematic reviews?

Assignment 2: Required Assignment 2—IT Strategic Planning

Using a company of your choice, determine the strategic business goals. Develop an IT strategy that aligns to the business goals. You can use the balanced scorecard approach or you can develop your own method for defining and aligning the IT strategy to the business strategy. You will need to cite at least two credible sources that will provide justification for your strategy or approach.

Finding a company: If you are employed, it is best for you to use your current place of employment so you can practically apply the concepts in this assignment. Alternatively, you can use a nonprofit organization or any other organization you may have ties or access to.

Your IT strategy should include:

  • Business Goals and Objectives
    • List at least two or three specific business goals for the next year or two with an explanation of the business’ rationale for the goal.
    • List an objective for each business goal to explain what the company needs to do to achieve the goal.
  • IT Strategies aligned with business goals and objectives
    • List one or two IT strategies for each business goal
      • These should be recommended strategies that would enable the business goals and objectives to be achieved.
      • If your company already has IT strategies, you may provide an analysis and justification or new recommendation for each of the strategies
  • Organizational Structure
    • Describe the structure of the organization including:
      • Organizational design
      • Organizational culture
      • Leadership factors (the types of leadership displayed within the company)
      • Business processes
      • Physical layout (i.e., how many offices, virtual offices, international locations)
  • IT Infrastructure
    • Describe the current IT infrastructure including:
      • Hardware
      • Software
      • Network
      • IT resources
    • This should be a high-level overview
  • IT Challenges
    • Describe internal and external challenges IT faces in meeting the business needs
    • Include social and/or ethical considerations
  • Risks
    • Describe risks IT might face when implementing change

As you develop your strategy, you may want to consider the following:

  • An effective IT strategy will enable the business operations, helping business leaders achieve business goals more efficiently and effectively.
  • Focus on top priorities. Take the time to understand organizational issues, how they are related, and how they affect performance. Determine what will happen if nothing changes and where IT can have the greatest, positive impact.
  • Identify how IT supports business processes. The business is relying on their capabilities to achieve the business goals. The IT strategy should include a plan for supporting those business capabilities, creating efficiencies, and competitive advantage for the business.
  • Define IT priorities and governance for IT. Without priorities and a process for IT governance in place, progress will be slowed. An agreed upon plan for governing IT will enable the decision process.

Understand the challenges. Improving processes and increasing efficiencies through IT implementation also requires behavioral and sometimes organizational change. With these changes come inherent challenges. Understanding and planning for those challenges will increase the chance of success in meeting the business goals.
